How can I load this relationship in a template.
That looks like a very old association that is not used anymore, anyway - the way to find what you need is to Explore the object - right click -> Explore - you will then see the associations that can be used in the Excel Import / Export. Export the one you have and then that file will be the template to import any new ones.
The Green and Yellow folders are the MetaAssociation ends you will need in the export.
