Why we can´t add risk to diagrams?

Hi community, question. 

I'm trying to add some risk at Business Process and Organizational Process diagrams, but I can´t, I'm clicking the risk element and add it, i type its name and clic create, but nothing happens.  I'm attaching an image related to this situation, it shows the license I'm using, and also the configuration of hopex. 

What could be the reason I can´t add risk?

we do not seem to have this issue on standard versions, which would indicate some kind customization ?

the 'description' attribute length is different form what the error above indicates :

the part "Invalid length 1051 on MetaAttribute Description" is new for me.

It works fine for me with Process Designer profile in V4 CP04 - what version are you using and did you check the error logs?
